Required Documents for Assessments or Orientation

All CAPS, Minors, Intermediate, and Juniors players must being hard copies of the following documents to Assessments:


Returning players who have completed district verification in a previous year:

New Players or players not verified by the district in a previous year, please bring the following required documents:

  • A copy of the player's birth certificate
  • A completed Medical Release Waiver Form
  • An extra copy of the completed Medical Release Form
  •  Player and Parent Code of Conduct
  •  
  • For residency verification there are three options: home address, school address, or waiver:
  • If your player will be using their home address for PLL eligibility, then bring three Proof of Residency Documents. These documents will be kept by the league so please bring a copy of anything for which you want to keep the original.)
  • If you player will be using their school address for PLL eligibility, then bring one copy of the completed and signed School Enrollment Form.
  • If you player does not live nor attend school within the PLL boundary, email the Player Agent at [email protected] to initiate the Waiver Process.  Please note that Waiver Requests are subject to approval by Little League and coordination with Peninsula Player Agent is required.
Tee Ball and Rookies players need to bring to orientation all the paperwork listed above, except that only one proof of residency is required if using the player's home address.  A school form or waiver request is also acceptable.
